I hereby release, waive, and hold harmless Samurai Action Studio Inc., and its officers, employees, instructors, agents, and representatives, from any and all claims, demands, causes of action, liabilities, damages, losses, costs, or expenses arising out of or related to my participation, or the participation of any minor under my legal custody, in stunt training, action-performance training, martial arts instruction, physical conditioning, workshops, classes, events, or related activities, except to the extent such a release is prohibited by applicable law.

I understand that stunt training, martial arts, physical conditioning, and action-performance activities involve inherent risks, including, but not limited to, falls, collisions, physical contact, equipment-related injuries, and other risks that may result in serious bodily injury, permanent disability, or death. I knowingly and voluntarily assume these inherent risks associated with participation.

If the participant is a minor, I represent that I am the participant's parent or legal guardian, or otherwise have legal authority to provide this consent. I give permission for the minor to participate in the activities and authorize reasonable emergency medical treatment if, in the judgment of appropriate personnel, such treatment is necessary due to accident or illness.

I understand that Samurai Action Studio Inc. has no obligation to supervise my child before or after scheduled class or activity times. I accept responsibility for arranging appropriate supervision and transportation outside scheduled program times and release Samurai Action Studio Inc., its officers, employees, instructors, agents, and representatives from claims arising from the lack of supervision outside those scheduled times, except to the extent prohibited by applicable law.

I grant Samurai Action Studio Inc. permission to photograph and/or record video of me or my child during classes, workshops, rehearsals, performances, and events, and to use those images or recordings for promotional, educational, marketing, social-media, and instructor-training purposes without compensation, unless I revoke this permission in writing for future use.
